ORANGE, Calif. (KTLA) -- A 41-year-old woman has been arrested in connection with a hit-and-run crash that severely injured a 4 year old girl.

Sarwat Syed of Ladera Ranch was arrested Monday night and released from Orange County Jail on $50,000 bail.

CHP officers were able to identify Syed as the driver with the help of a partial license plate and an insurance claim she made on the day after the accident, according to CHP officer John Patterson.

4 year old Ava O'Connor was hurt on June 20 when the car she was riding in went off the northbound San Diego (405) Freeway near Laguna Canyon Road.

Authorities say Annette Argo, 40 was driving her two children -- 4-year-old Ava and 9-year-old Gaven -- home from Pretend City, a children's play area in Irvine, when she was hit by an SUV and went off the road.

The driver of the SUV took off, leaving the family in a ditch. Shortly after, a good samaritan stopped to call 911 and also called Robert O'Connor -- Ava's father.

O'Connor rushed to the hospital to find his daughter had suffered severe injuries and was in the intensive care unit.

Doctors spent nine hours operating on her to fix her jaw, upper palette, eye sockets and nose. She also underwent a tracheotomy.

Gaven suffered a broken clavicle. Their mother suffered extensive damage to her right leg.

O'Connor and his wife, Erica, offered the $10,000 reward in the case. They say they are relieved that an arrest has been made and thankful to the CHP for their investigation.

Little Ava is doing much better and recovering at home.

Syed has been charged with felony hit and run.
